Skilled Nursing Care and Rehabilitation
The Trinity Terrace Health and Rehabilitation Center offers an array of rehabilitation services, short-term skilled nursing care for individuals recovering from illness or injury, and long-term care for individuals needing nursing services for longer periods.
One does not need to be a Trinity Terrace resident with a Continuing Care contract to access these services; our Direct Admission program is open to the general public.
Our Health and Rehabilitation Center is Medicare-certified and our professional staff members are caring, compassionate, and available around-the-clock, seven days a week. What really sets the skilled nursing care at Trinity Terrace apart is our philosophy of providing services and care that are tailored to individual needs. So wherever residents are in the care continuum, Trinity Terrace offers personalized programs that enhance health and wellness.
Our staff of licensed and certified professionals review each resident’s Plan of Care with the resident’s private physician. They also work with visiting doctors, dentists, and podiatrists in order to customize each resident’s program of care.
