Our specialized Skilled Nursing and Rehabilitation program, called Pathways to Progress, is staffed with professionals ready to assist residents in your transition from hospital to home. Pathways’ sub-acute and rehabilitation program offers a full range of in-patient medical care, nursing services, and advanced rehabilitation programs. The program focuses on helping individuals progress quickly and successfully in their recovery and rehabilitation.
With an interdisciplinary team consisting of physicians, nurse practitioners, nurses, pharmacists, licensed therapists, and dietitians, every individual’s care needs will be met, as will their social and emotional needs.
Upon admission, the social worker will begin the discharge planning process and assist in the coordination of any necessary services individuals may need upon their return home.
Capitol Lakes also offers long-term nursing care for people who are unable to return home. Our professional staff provides 24-hour care to individuals who are no longer able to care for themselves due to physical, emotional, or cognitive conditions. The services we offer include personal care, specialized diets, medication management, restorative therapy, social services, spiritual services, and recreational and leisure-time activities.
We are an approved provider for many HMOs and private insurance carriers, and we are Medicare Certified.
