Capitol Lakes
Benefits Package

Medical Benefits available beginning the first of the month following 90 days of full or part time (20+ hours per week) employment.

Medical Benefits: (You may choose one of the two plans.)

Physicians Plus Tiered Co-Pay
$600 deductible for Single plan and $1,200 deductible for Family plan. Office visits are $22 for general practitioners and $44.00 for specialists. Prescriptions are $10.00 generic, 30% for Formulary brands and 50% for Non-formulary brands with $1,600 out-of-pocket max for prescriptions. No calendar year out-of-pocket max for the medical.

Physician Plus Custom
$250 deductible for Single plan and $500 deductible for Family plan. Office visits 20% in network. Prescriptions are $10.00 generic, 30% for Formulary Brands and 50% for Non-formulary brands with a $1,600 maximum for prescriptions. Max out of pocket: $500.00 single/$1000.00 family.

Dental Benefits available beginning the first of the month following 90 days of full or part time (20+ hours per week) employment.

Dental Benefits: (You may choose one of the three dental plans.)

American Dental
Annual max. coverage $1500.00. Preventive exams, cleaning and x-rays covered in full. Basic services covered in full. Orthodontia $2000.00 benefit.

Delta Standard
Annual max. coverage $500.00. $25.00 annual deductible for single and $75.00 family. Preventive exams, cleaning and x-rays covered 20%. Basic services 20%. Orthodontia not covered.

Delta Extra
Annual max. coverage $2000.00. Preventive exams, cleaning and x-rays covered in full. Basic services covered in full. Orthodontia $1500 benefit.

Flexible Spending Account
Insurance Premium Contribution Each year, during open enrollment, employees may elect to use pre-tax dollars to pay for their insurance premiums. (Medical and dental)

Medical Reimbursement
Each year, during open enrollment, employees may elect to use pre-tax dollars to pay for medical costs not covered by their medical or dental plan. This money can also be used by employee dependents. Dependents do not have to be enrolled in the PRS benefit plans to us pre-tax dollars for medical expenses.

Dependent Care Expenses
Each year, during open enrollment, each employee may elect to use pre-tax dollars to pay for Dependent Care expenses. If you choose to use this benefit, your child care provider will have to declare their income to the IRS.

Life Insurance
Group Term Life Insurance is available to employees who have worked less than 5 years and qualifies for insurance benefits, are entitled up to $10,000.00 dollars of life coverage as an employee benefit. Employees who have worked for 5 or more years are qualified for up to $25,000. Employees may also purchase supplemental coverage for themselves, their spouse or children at an additional cost.

403(b) Retirement Plans

Voluntary 403 (b) Plan
Employees may elect to use pre-tax money and invest in the Capitol Lakes voluntary 403(b) retirement plan. The Summary Plan Description is available in Human Resources.

Roth 403 (b) Plan
Employees may elect to use post-tax money and invest in the Capitol Lakes voluntary 403(b) retirement plan. The Summary Plan Description is available in Human Resources.

Available after the first year:

Matching 403 (b) Plan
Employees who are: over 21 years of age, have worked at least 1,000 hours, and have been employed with Capitol Lakes for at least 1 calendar year are eligible for up to a 4% match. Capitol Lakes will match up to 4% of your pre-tax contribution. The Summary Plan Description is available in Human Resources.

Pension
Employees are automatically enrolled in the Capitol Lakes employer funded pension plan after the employee has worked one full year (at least 1,000 hours). Capitol Lakes will contribute 3% to the employee’s pension fund. After participants have been enrolled in the plan for three years, they are fully vested. ADDITIONAL BENEFITS

Available immediately:

Jury Duty Pay
Full-time and Part-time employees who are called for jury duty will be permitted to meet this civic obligation. Employees will be paid their regular wage for scheduled work missed in exchange for signing over to Capitol Lakes any jury duty fees received from the court. Temporary and on-call employees are not eligible for paid time off for jury duty.

Personal Leave
Employees who are part-time or full-time may request a leave of absence without pay for a maximum of 30 calendar days.

Available after the first three months:

Sick Leave
Sick leave is provided to protect against an employee’s loss of income due to illness or injury. Upon completion of the trial service period, all full-time and part-time employees are eligible for sick leave benefits.

Holidays
Capitol Lakes provides six paid holidays to full-time and part-time employees who have successfully completed their trial service period.

Funeral Leave
An employee will be granted up to three days leave to attend the funeral and/or to make household arrangements due to a death in the immediate family. This leave will be charged against the employee’s accumulated sick leave. If sick leave is not available, vacation leave may be used or it will be treated as an unpaid leave. If more than three days are required, vacation leave maybe be used or it will be treated as an unpaid leave.

Available after the first year:

Vacation Days
Employees who are full-time or part-time and have completed 12 months of active employment will be eligible for annual vacation time with pay on the anniversary date of their latest date of hire at the following rates.

Vacation hours are calculated according to the employee’s hours worked in the preceding year.

Floating Holiday
One additional holiday, a floating holiday, is provided to full-time and part-time employees who have completed one-year of continuous active service.

WELLNESS BENEFITS

Aquatic And Wellness Center
All Capitol Lakes employees are welcome to use the Aquatic and Wellness Centers during specified times. Employees will need to complete a member’s packet available at the Wellness Center. Schedules are posted. Residents have first priority on all equipment at all times in the wellness center. Employee Discount: 20% discount on services provided by the Aquatic & Wellness Center. Memberships are month-to-month.

Tuition Reimbursement
Full and part time employees may be eligible after the first 6 months of employment for tuition reimbursement for education for the RN or LPN program. Up to $1,500 a year. Pro-rated for part-time employees.
